Basic medical hx: Alexis has had two uncomplicated pregnancies and births. She had her tonsils removed as a child and her wisdom teeth removed in high school.  

Date and result of most recent pap smear: 10/8/26-Pending, April 2024- normal

Has had the Flu shot and is willing to get it again. Not willing to get Covid vaccine.

About Alexis

Alexis is a 27 year old Biracial female who is hoping to become a surrogate. Alexis lives in Albertville Alabama with her two children. The closest airport is Huntsville Airport.

Alexis is 5’4” and weighs 127 pounds and has a BMI of 21.8. She speaks English and is a US Citizen. Alexis says that she is an introvert, loves to go fishing, bowling, and hiking with her daughters.

Alexis does not have any insurance and will require a policy

Alexis works full time in Customer Services/ Shipping. She started working with them in June 2025 and typically works 40 hours per week. She makes approximately $20/hour. 

Alexis graduated from Albertville High School in Albertville, Alabama in 2017. She did not attend college.

Alexis has never been a surrogate and has never donated eggs. Alexis has had two pregnancies and has never had an abortion or a miscarriage. Alexis has never placed a child for adoption and does not have any deceased children. Alexis reports that she has had easy pregnancies with both of her children. Both children were born vaginally. Her daughter was born on 7/4/2017 when Alexis was 18 years old. Her daughter weighed 7 pounds 4 ounces at birth. She was born at 37 weeks.. Her second daughter was born at 37 weeks and weighed 8 pounds. Alexis denies any pregnancy or delivery complications. Alexis has custody of both of her children. Neither child has any physical, psychological, or behavioral problems. Alexis has never gone through fertility treatment to become pregnant. She has never delivered a stillborn baby.

Alexis does have regular menstrual cycles. Her cycles are approximately 28 days apart and last for 7 days on average. She has the Nexplanon for birth control. 

Alexis’s last pap smear was conducted in April 2025 and was normal. She has never been treated for HIV/AIDS or any STD. Alexis has never suffered from depression and has never been prescribed psychiatric medications. Alexis has only been hospitalized for the births of her two children. Her blood type is unknown.

Alexis does not smoke cigarettes or use any smokeless tobacco. Alexis denies any drug use and has never used any illegal drugs. Alexis has never been diagnosed with any of the following: gonorrhea, hepatitis C, HIV, syphilis, hepatitis B, herpes, genital warts/sores, or anemia. Alexis likes to stay active and has never had an eating disorder. She states that she does not drink.

Alexis does not have any legal claims/cases pending at this time. Alexis has never been arrested or had conflicts with the law, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program, or declared bankruptcy.

Alexis has discussed becoming a surrogate with her friends, and family members and has a great support system. Alexis has no concerns about sharing her surrogacy journey with her children and Alexis states that there isn’t anyone that she is aware of that is against her desire to become a surrogate.

Alexis plans to save the money she earns from being a surrogate.

Alexis is very open to whom she is willing to carry for. She is open to carrying for a heterosexual couple, homosexual couple, a family of any race, and a family with children or without children, Christian, non-Christian, and single parents.

Alexis is ok with having selective reduction/termination/abortion for any severe medical/genetic abnormalities or if her life is in danger. She will undergo a selective reduction if medically advised by the treating physician. She will not terminate for Down Syndrome, Cleft Palate, or any cosmetic reasons. Alexis is ok with an amniocentesis if deemed necessary by a doctor and the IPs are aware of the risks. Alexis states that she is ok with the intended parents being in the room and being at the hospital so they can take custody of the baby as soon as he/she is born. She is open to communication after delivery if the intended parents desire it. Alexis is open to the intended parents joining her at doctor appointments (in person if local and via Zoom or Facetime if not able to make it in person) and taking pictures throughout their journey together.
