Date and result of most recent pap smear: 2021. Has a pap smear appointment in 2 weeks

 Is not willing to receive the flu vaccine or the Covid vaccine.

About Amanda

Amanda is a 37 year old, married, caucasian female who has no preference in the  heterosexual family that she carries for. Amanda lives in Roseburg, Oregon with her husband, Froylan “Froy”, and their three children.  

Amanda is 5’9’’ and weighs 174 pounds and has a BMI of 25.7. She speaks English and is a US Citizen. Amanda describes herself as outgoing. She stated that she is a busy mom of three who loves to spend time with her best friend, being with her family and her plants. Amanda said she has a mini jungle in her home. 

Amanda does have insurance through her husband’s employer. It will be reviewed to see if it will cover a surrogacy pregnancy. 

Amanda has been a hair stylist for the last 12 years. She works 20-25 hours a week and averages around $74 a client with a three month average of $5800. Froy works maintenance at a community college and makes a gross pay of $1855.83 every two weeks.

Amanda graduated from Roseburg High School in 2007. She did not attend college but did attend a trade beauty school. 

Amanda has never been a surrogate or an egg donor. Amanda has had three pregnancies and has never had an abortion or a miscarriage. Amanda has never placed a child for adoption and does not have any deceased children. Her first child, a daughter, was born March 29, 2013. She weighed 6 pounds 2 ounces and was born at 36 weeks. Amanda stated that due to her size at 300 pounds she developed pre-eclampsia and was induced at 36 weeks. Her second child, a son, was born on January 7, 2016. He weighed 7 pounds 8 ounces and was born at 41 weeks. Her last child, another son, was born on May 2, 2021. He weighed 8 pounds 2 ounces and was born at 39 weeks. All three children were born via vaginal delivery and are healthy with no psychological or behavioral problems. Amanda is not breastfeeding. Amanda has never gone through infertility treatments to become pregnant. 

Amanda has a regular menstrual cycle that last for 3-6 days. Amanda is not currently on birth control. Froylan had a vasectomy. Amanda states that her mother did not take diethylstilbestrol (DES) while pregnant with her to the best of her knowledge.

Amanda’s last pap smear was in 2021. She has a new appointment in 2 weeks. She has never had an abnormal pap smear. She has never been treated for HIV/AIDS. Amanda has never suffered from depression. She has never been prescribed any psychiatric medication. She has never seen any mental health professionals. Amanda did have the following surgeries, in 2015 she had her gallbladder removed. In 2016 she had a stomach sleeve done in Mexico and in August 2025 she had a revision to bypass because of a hiatal hernia.

Amanda nor Froylan smoke or vape. Amanda does not drink alcohol.  Amanda denies any illegal drug use. Amanda has never been diagnosed with any of the following: hepatitis C, HIV, hepatitis B, herpes, genital warts/sores, gonorrhea or syphilis. 

Amanda nor Froylan have any legal claims/cases pending at this time. Neither of them, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program. Amanda has never been arrested or had conflicts with the law. Froylan had an arrest related to alcohol. He went through a treatment program (2024) and is now sober. Amanda nor Froylan have never filed for bankruptcy. 

Amanda has discussed becoming a surrogate with Froylan and her friends and family members and has a great support system. Amanda has no concerns about sharing her surrogacy journey with her daughter. Amanda states that there isn’t anyone that she is aware of that is against her desire to become a surrogate. 

Amanda is willing to carry for any heterosexual family located in the US or internationally. Amanda is willing to give up pork or beef products through the journey for a family's religious beliefs. Amanda said that she has thought about surrogacy for years but her BMI has always been too high, now that she is at a healthy weight she would like to pursue surrogacy. Amanda currently has a friend that has suffered multiple miscarriages and is currently working with a fertility clinic as well as two clients that have been surrogates. 

Amanda is only willing to terminate if her life is in danger. She is willing to carry twins and triplets. She is not open to selective reduction in the event of a triplet pregnancy. She is willing to transfer more than one embryo at a time. She is comfortable with the Intended Parents taking pictures during this journey. She is comfortable with the intended parents being at doctor’s appointments,and being in the delivery room. Amanda stated that she would like a lot of communication throughout the journey. She would like to stay in touch after the delivery as long as the Intended Parents feel the same way.  

Amanda is not willing to receive the flu vaccine or the Covid vaccine.
