Basic medical hx: Overall healthy. Cannot take birth control due to side effects of migraines and abdominal pains

Date and result of most recent pap smear: 11-19-24 negative

Is willing to receive the flu vaccine but Is not willing to receive the Covid vaccine.

About Autumn

Autumn is a 21 year old, caucasian female who has no preference in the couple she will carry for. Autumn lives in Greenwood, South Carolina with her daughter. Autumn is in a committed relationship with a man named Zay but he does not live with her. Her date of birth is 9/10/2003.

Autumn is 4’11” and weighs 160 pounds and has a BMI of 32. She speaks English and is a US Citizen. Autumn said that she likes to sing and describes herself as socially awkward. Her favorite thing to do is spend time with her daughter. 

Autumn does not have insurance. A policy will need to be purchased to cover the pregnancy. 

Autumn currently works for Doordash and makes around $800 a week. 

Autumn attended Greenwood High School but did not graduate. She received her GED.

Autumn has never been a surrogate and has never donated eggs. Autumn has had one pregnancy and has never had a miscarriage or an abortion. Autumn has never placed a child for adoption and does not have any deceased children. Her daughter was born on October 31, 2021. She weighed 5 pounds and 8 ounces and was born at 39 weeks 3 days and was a vaginal birth. In her medical records, starting around 37 weeks and 3 days there is mention of fetal growth restriction. No mention of that before 37 weeks and 3 days. Autumn states that there were no complications. Autumn has custody of her daughter. Her daughter is healthy with no psychological problems. Autumn is currently not breastfeeding. She has never gone through fertility treatments to become pregnant. 

Autumn has a regular menstrual cycle that lasts 4-5 days. Autumn states that her mother did not take diethylstilbestrol (DES) while pregnant with her to the best of her knowledge.

Autumn’s last pap smear was done in 11-19-24 and was normal. She has never been treated for HIV/AIDS. She has never been told she has HPV and we do not see it in her records. Autumn has never been diagnosed with any of the following: hepatitis C, HIV, hepatitis B, herpes, genital warts/sores, syphilis or gonorrhea. She has been diagnosed with chlamydia and trichomoniasis. She completed her treatment course and has not had any issues since. She had ovarian cysts several years ago but has not had any issues since. 

Autumn has never suffered from depression. She has never been prescribed medication. She has never seen a psychologist or psychiatrist. She has only been hospitalized for childbirth and Covid. 

Autumn does not smoke cigarettes. Autumn denies any drug use and occasionally drinks alcohol socially. Autumn denies any illegal drug use. 

Autumn does not have any legal claims/cases pending at this time. She has never been arrested or had conflicts with the law, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program, or declared bankruptcy. 

Autumn has no preference in the family she will carry for. She is open to being a surrogate for a family that lives outside of the USA. She is willing to match with a family that is a different race or a different religion. Autumn is willing to give up pork products if matched with a family of a different religion. Autumn stated that she had friends that have suffered from infertility and she wants to help another family feel that heartache.

Autumn is willing to have a selective reduction, abortion, or an early termination for any reason that the Intended parents choose. She is willing to have an amniocentesis done. She is willing to carry twins and triplets if they happen naturally but will not transfer more than one embryo. She is comfortable with the Intended Parents taking pictures during this journey. She is comfortable with the intended parents being at doctors appointments, and being in the delivery room. Autumn is open to any amount of communication that the Intended Parents want during and after the journey. 

Autumn is willing to get the flu vaccine but she is not willing to get the Covid Vaccine.

Hi there! I'm Autumn.

I’m so excited to be on this journey and can’t wait to meet the family I’ll be lucky enough to support. Becoming a surrogate has been a dream of mine for a long time—it’s something I’ve always felt deeply called to do. The idea of helping someone experience the joy of parenthood is truly special to me.

My hope is to build a genuine connection with the family I carry for, and I’d love for that connection to last even beyond the birth, if that’s something we both want. This process is so meaningful, and I want it to feel personal, warm, and full of trust.

Being a mother myself has changed my life in ways I never imagined. My daughter has been my greatest gift, and she’s helped me through some of my hardest moments. If I can help give someone else that same kind of joy and love, I’ll feel incredibly grateful.

The experience of bringing a child into the world is so wholesome and beautiful—I’m honored to be part of that for someone else.
