Basic medical hx: Gastric bypass July 17, 2024, gallbladder 11/14/25, thyroidectomy 2021 for cosmetic issues then pathology showed cancerous lesions, wisdom teeth at 18 years old

About Lauren
Lauren is a 36 year old, married, caucasian female who has no preference for the couple she will carry for. Lauren lives in Crown Point, Indiana with her husband Brian and their four children.
Lauren is 5’5'' and weighs 178 pounds and has a BMI of 29.6. She speaks English and is a US Citizen. Lauren describes herself as bubbly. She loves the arts, reading, cross country skiing, ice skating, and the snow. Her favorite thing to do is be with her husband and children.
Lauren does not have insurance. A policy will need to be purchased for her.
Lauren works as an HR specialist with a mental health clinic. She works 20 hours a week and makes $19 an hour. She will be going full time March, 2026. Brian works as the director of student central at a University. He makes $95,000 a year.
Lauren graduated from Liberty Christian School in 2007. She later attended SCAD where she received her Bachelors in 2011. She later received her Masters from SCAD in Human Resource Management in 2023.
Lauren has had four pregnancies and has never had an abortion or miscarriages. Lauren has never placed a child for adoption and does not have any deceased children. Her first child, a daughter, was born on April 9, 2016. She weighed 7 pounds 7.2 ounces and was born at 41 weeks 4 days. Her second child, another daughter, was born on January 23, 2018 She weighed 8 pounds 9 ounces and was born at 41 weeks 4 days. Her third child, a son, was born on March 4, 2019. He was born at 41 weeks 6 days and weighed 7 pounds 14.3 ounces. Her last child, another daughter, was born on March 20, 2022. She weighed 7 pounds 14 ounces and was born at 39 weeks 4 days. All the deliveries were vaginal deliveries. Lauren stated that the only complications were in her last pregnancy. Lauren says she had a headache x 1 day and induced her. BP on that visit was 128/86 with HR of 79. Lauren is not breastfeeding. She has never gone through fertility treatments to become pregnant. Lauren and Brian have custody of all four children.
Lauren has a regular menstrual cycle that last for 6 days. Lauren is not currently on birth control. Lauren states that her mother did not take diethylstilbestrol (DES) while pregnant with her to the best of her knowledge.
Lauren’s next pap smear is scheduled for February 5, 2026. Her last pap smear was normal. Lauren has never had an abnormal pap smear. She has never been treated for HIV/AIDS. Lauren has never suffered from depression. She has suffered from anxiety and was prescribed medications. She is no longer taking those. Lauren sees a therapist because she feels it is important since she works in a mental health clinic and is a mom of four. She thinks it is important to maintain mental health. She has never suffered from postpartum depression. Lauren has only been hospitalized for childbirth. She has had the following surgeries: gastric bypass July 17, 2024, gallbladder 11/14/25, thyroidectomy 2021 for cosmetic issues then pathology showed cancerous lesions, wisdom teeth at 18 years old. Lauren is allergic to latex and nickel.
Lauren does not smoke cigarettes. Lauren does not use any smokeless tobacco. No one in Lauren’s home smokes cigarettes or uses any smokeless tobacco. Lauren does occasionally drink alcohol but never during pregnancy. Lauren denies any illegal drug use. Lauren has never been diagnosed with any of the following: hepatitis C, HIV, hepatitis B, genital warts/sores, gonorrhea or syphilis or herpes.
Lauren does not have any legal claims/cases pending at this time. She have never been arrested or had conflicts with the law, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program. Lauren has never ever filed for bankruptcy.
Lauren has discussed becoming a surrogate with her friends and family members and has a great support system. Lauren has no concerns about sharing her surrogacy journey with her children. Lauren states that there isn’t anyone that she is aware of that is against her desire to become a surrogate.
Lauren is open to carry for any family. She said that she is willing to be a surrogate for a family that lives outside of the USA. Lauren does not eat pork and will not eat pork due to a family's religious views. Lauren stated that she wants to become a surrogate because “her friend had PCOS and struggled with infertility but went on to have 2 children. I considered carrying for her before she got pregnant and it would have been such an honor. That’s what inspired all of this!”
Lauren is willing to have a selective reduction/early termination or an abortion for any reason if the embryos are genetically tested, but will not terminate for cleft palate, cleft lip, missing a limb She is only willing to transfer one embryo at a time. She is comfortable with the Intended Parents taking pictures during this journey. She is comfortable with the intended parents being at doctor’s appointments,and being in the delivery room. Lauren stated that she is okay with as much contact as the Intended Parents need throughout the journey. She stated that she would like communication after birth if the Intended Parents want that as well.
Lauren is willing to receive the flu vaccine, the Covid vaccine and any other vaccines recommended by the doctor.
