Basic medical hx: Overall healthy. Remote history of taking Lexapro, has not been taking for several years- situational due to ex spouse custody battle. Says she has not talked to the ex spouse for 5 years, kids are grown now.
Date and result of most recent pap smear: 1/28/26 (pending), 11/30/2021- normal. LEEP procedure in 2011. Received clearance by OB doctor. Pending pap results from 1/28/26.
Is willing to get the flu vaccine and all other vaccines needed except the Covid vaccine.
About Rachael
Rachael is a 39 year old, single, caucasian female who has no preference in the family she will carry for. Rachael lives in Springfield, Missouri with her four daughters.
Rachael is 5’4” and weighs 161 pounds and has a BMI of 276. She speaks English and is a US Citizen. Rachael describes herself as outgoing and shy depending on the situation. She loves to sew, home renovations, refinishing furniture, going floating and anything outside. Her favorite thing to do is spend time with her family.
Rachael does not have insurance. A policy will need to be purchased for her to cover the surrogacy pregnancy.
Rachael does several jobs throughout the year. She is a hobby breeder of dalmations, does resale and Doordash. She works 40 hours a week and makes 25/hr.
Rachael graduated from Branson High School in 2005. She later attended college but did not finish. She has no plans to pursue any more degrees.
Rachael has never been a surrogate and has never donated eggs. Rachael has had four pregnancies and has never had an abortion or a miscarriage. Rachael has never placed a child for adoption and does not have any deceased children. Her first daughter was born on April 29, 2004. She weighed 7 pounds 5 ounces. Her second daughter was born on March 21, 2006. She weighed 6 pounds. Her third daughter was born on August 28, 2007. She weighed 6 pounds 3 ounces. Her last daughter was born on May 23, 2025. She weighed 8 pounds 3 ounces. All four girls were born at 40 weeks via vaginal delivery. Rachael states that there were no complications with the pregnancy or delivery. Rachael has custody of her children. Rachael is not breastfeeding and has never gone through fertility treatments to become pregnant.
Rachael has regular menstrual cycles that last 4-5 days. Rachael states that her mother did not take diethylstillbestrol (DES) while pregnant to her to the best of her knowledge.
Rachael’s last pap smear was conducted 11/30/2021 and was normal. She has a new appointment scheduled for 1/28/2026. She had a LEEP procedure in 2011. She has never been treated for HIV/AIDS. Rachael has never suffered from depression. She did struggle with anxiety during her court case for custody. Rachael did see a mental health professional during this time, she was also prescribed psychiatric medications for anxiety but did not take them as they were not effective for her. Rachael has only been hospitalized for childbirth. She is allergic to Iodine and has surgery to remove her wisdom teeth.
Rachael does not smoke cigarettes or use any smokeless tobacco. Rachael denies any drug use. Rachael has never used any illegal drugs. Rachael does not drink alcohol. Rachael has never been diagnosed with any of the following: gonorrhea, hepatitis C, HIV, syphilis, hepatitis B, herpes, genital warts/sores, or anemia. Rachael has never had an eating disorder.
Rachael does not have any legal claims/cases pending at this time. She has never been arrested or had conflicts with the law, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program, or declared bankruptcy.
Rachael has discussed becoming a surrogate with her daughters, friends, and family members and has a great support system. Rachael has no concerns about sharing her surrogacy journey with her children and states that there isn’t anyone that she is aware of that is against her desire to become a surrogate.
Rachael has no preference in who she carries for. She stated that she was open to anyone and everyone. She said that she is willing to be a surrogate for a family that lives outside of the USA. Rachael is willing to give up pork products and beef products through the journey for a family's religious beliefs. Rachael stated that she does not eat a lot of meat in her diet. Rachael wants to become a surrogate because she is adopted and knows that families grow through all options. She loves to help people and cannot wait to help make a difference in someone's life through surrogacy.
Rachael is willing to have selective reduction or an abortion for any reason that the Intended Parents choose. She is willing to carry twins and triplets. She is only willing to transfer one embryo and is willing to do a selective reduction for a triplet pregnancy. She is comfortable with the Intended Parents taking pictures during this journey. She is comfortable with the intended parents being at doctors appointments, and being in the delivery room. Rachael said she is open to any amount of communication that the Intended Parents was during the pregnancy. She would like to remain in contact after the birth if the Intended Parents feel the same as well.
Rachael is willing to receive the flu vaccine and all other recommended vaccinations, except the Covid vaccine. She is not willing to receive the Covid vaccine.
