Season allergies/asthma-inhaler as needed. Not daily use. Has never been hospitalized for that.

Thyroid removed-takes levothyroxine. Precancerous cells were found and her sister had thyroid ca. They took out the thyroid out of precaution and it was found to be non-cancerous when they removed it. 

Current medications: HSV-1 valtrex as needed

IUD/Provera hx: n/a, tubal

Basic medical hx: Tabatha experienced some endometriosis at a young age. She had an exploratory laparoscopy completed around 2013 to remove some mild scar tissue on the outside of her uterus and she has never had any issues since.

Date and result of most recent pap smear: over 2 years ago, normal, will obtain a new one

Fully vaccinated for the Covid Vaccine

Not willing to get the flu vaccine

About Tabatha

Tabatha is a 32 year old white female who is hoping to become a surrogate. Tabatha lives in Aiken County, South Carolina with her husband, Travis, and their two children. The closest airport is Augusta Regional Airport.

Tabatha is 5’9” and weighs 180 pounds and has a BMI of 26. She speaks English and is a US Citizen. Tabatha says she is a full time mom who is loving and caring. She loves photography and reading books.

Tabatha has United Healthcare insurance through her employer and Blue Cross Blue Shield Insurance through her husband’s insurance. We will be sending it off to IFI for assessment. 

Tabatha works full time as an accountant at a local lumber company. She started working with them 6 years ago and typically works 40 hours per week. She makes approximately $28.75/hour. Her husband is a firechief and makes approximately $2,750 per week. 

Tabatha graduated from Woodmont High School in Piedmont, South Carolina in 2012. She went to Andersen University and graduated with a Bachelors Degree in Organizational leadership. 

Tabatha has never been a surrogate and has never donated eggs. She has had four pregnancies and has never had an abortion. Tabatha has never placed a child for adoption and does not have any deceased children. Tabatha reports that she has had easy pregnancies with both of her children. Both children were born via c-section. In 2015, she experienced a very early 6 week miscarriage which required no intervention to resolve itself. Her first son was born on 3/4/2021 at 37+6 weighing 7 pounds 6 ounces. In 2023, she experienced a chemical pregnancy which required a D&C. On 8/28/2025, she delivered her second son at 37+5 weeks weighing 7 pounds 6 ounces. Tabatha denies any pregnancy or delivery complications. Travis and Tabatha have custody of both of their children. Neither child has any physical health problems, any psychological or behavioral problems. Tabatha has never gone through fertility treatment to become pregnant. She has never delivered a stillborn baby.

Tabatha does have regular menstrual cycles. Her cycles are approximately 28 days apart and last for 5 days on average. 

Tabatha’s last pap smear was conducted about 2 years and was normal. She will complete a new one. She has never had an abnormal pap. She has never been treated for HIV/AIDS or any STD. Tabatha has experienced a short period of postpartum depression after her first son. Her OBGYN prescribed her an antidepressant to have on hand in case she felt like she needed it but she states that she never took it and the depression resolved itself within a few weeks. Tabatha has had a D&C for the chemical pregnancy, 2 c-sections, a breast augmentation, and a laparoscopy for her endometriosis. Her blood type is B+.

Tabatha does not smoke cigarettes or use any smokeless tobacco. Tabatha denies any drug use and has never used any illegal drugs. Neither Tabatha nor her husband Travis have ever been diagnosed with any of the following: gonorrhea, hepatitis C, HIV, syphilis, hepatitis B, herpes, genital warts/sores, or anemia. Tabatha likes to be active and has never had an eating disorder. 

Neither Tabatha nor her husband have any legal claims/cases pending at this time. Neither have been arrested or had conflicts with the law, been convicted of a felony, been accused and/or convicted of child abuse or spousal abuse, lost custody of a child, been in a substance abuse program, or declared bankruptcy.

Tabatha has discussed becoming a surrogate with her husband and friends and has a great support system. She has no concerns about sharing her surrogacy journey with her children and states that there isn’t anyone that she is aware of that is against her desire to become a surrogate.

Tabatha plans to save the money she earns from being a surrogate.

Tabatha is very open to whom she is willing to carry for. She is open to carrying for a heterosexual couple, homosexual couple, a family of any race, and a family with children or without children, Christian, non-Christian, and single parents. Surrogacy has been on her heart for a long time and now that their family is complete they are excited to help a family in need.

Tabatha is ok with having selective reduction/abortion/termination for any extreme severe genetic/medical reason or if her life is in danger. She will not terminate for Down Syndrome, Cleft Palate, cosmetic reasons, or any non severe reasons. Tabatha is ok with an amniocentesis if deemed necessary by a doctor. Tabatha states that she is ok with the intended parents being in the room and being at the hospital so they can take custody of the baby as soon as he/she is born. She would love to receive updates after delivery if the intended parents desire it. Tabatha is open to the intended parents joining her at doctor appointments (in person if local and via Zoom or Facetime if not able to make it in person) and taking pictures throughout their journey together.
